Our Circle of Support
Each one of our patients is served by an interdisciplinary team of hospice professionals, ranging from hospice physicians and Nurse Practitioners to social workers and clergy. These specialists collaborate closely with one another to meet the patient and family’s needs, from preparing for an end-of-life experience with compassion and dignity through the bereavement process. Together, these team members form a circle of support dedicated to providing the best possible care to you and your loved ones. Watch the video above to learn more.

When you have a team caring for you that doesn’t just mean you’ve got three or four different types of people in your house; that means you’ve got a team that sits down together on a regular basis. In fact, it’s mandated that they meet at least every other week and talk about you and your care plan, and whether or not you’re having your needs met and how the family is dealing with this and what else do we need to provide.

It provides nursing support, psychosocial support with social workers, spiritual care with chaplains. We have CNAs, which are certified nurses aids who provide personal care. Volunteers. A big part of our team is our physician involvement and all of these people work together.

| Hospice Physicians | |
| Evercare has a board-certified hospice and palliative care physician on site at each location. Actively involved with patient care on a day-to-day basis, they work alongside the patient’s attending doctor and assist in providing comfort and symptom management. Hospice physicians are also part of a local leadership team and a national Medical Director’s team, helping shape the future of hospice care across the country. |

| Nurse Practitioners | |
| Our Nurse Practitioners have specialized training in pain and symptom management. All of our Nurse Practitioners are graduates of an approved Nurse Practitioner program, possess a current professional license and national certification and are skilled in the assessment and treatment of common problems of the terminally ill. Our Nurse Practitioners have some intangible qualifications as well—a passion for personal involvement and the desire to work from the heart and change lives. |

| Certified Nursing Assistants | |
| Certified Nursing Assistants work under the supervision of the Registered Nurse to increase the patient's comfort, maintain a safe environment, and help with personal care such as bathing, personal hygiene and grooming. They follow the plan of care in the patient's home, care center or assisted living setting. | |

| Volunteers | |
| Volunteers are an integral part of Evercare Hospice. Many volunteers work directly with patients and families, providing companionship and emotional support. This could be sharing hobbies, running errands, being a sympathetic listener, writing a letter, preparing a meal or providing caregiver support. Other volunteers contribute important organizational support and participate in program activities and help with events and celebrations. Another specialized area of volunteering is bereavement, where volunteers help families navigate the grief process during the first year of loss. |

| Chaplains | |
| Hospice chaplains help the patient and family explore spiritual questions. They do not represent a particular denomination or church and respect personal beliefs. | |

| Registered Nurses | |
| Hospice Registered Nurses work with the other members of the interdisciplinary team to implement the plan of care. They have specialized training in meeting the needs of patients and families facing terminal illness and bereavement. | |

| Therapists | |
| Therapists provide physical, occupational, speech and nutritional expertise. | |
